Individuals aged 16 and over who are applying for their first U.S. passport, or whose previous passport was lost, stolen, damaged, issued before age 16, or expired more than 15 years ago, must complete Form DS-11.
Applicants are required to bring a filled but unsigned DS-11 form to their appointment. They must also provide proof of U.S. citizenship such as a valid passport, birth certificate, Certificate of Naturalization or Citizenship, or Consular Report of Birth Abroad along with a photocopy. A recent color photograph measuring 2 by 2 inches with a white background is necessary.
A government-issued unexpired ID—either foreign or U.S.—and its photocopy are needed for identification purposes. Applicants must list their Social Security number on the DS-11 form if they have been issued one; those without a Social Security number will receive further instructions during the appointment.
Payment details can be found on the embassy’s website. If payment is made online, applicants should bring confirmation to the appointment. For in-person payments at the Consular Agency or U.S. Embassy, exact change in either U.S. dollars or Bolivianos or a U.S. credit card is required.
If an applicant's previous passport photo was taken when they were a baby or if their appearance has changed significantly since their last passport was issued, bringing age progression photos is strongly encouraged to help verify identity.
For applicants aged 16 and 17, it is strongly recommended that at least one parent accompany them to the appointment with valid government-issued identification and a photocopy.
