U.S. Embassy consular officers to offer citizen services in Lviv on October 15

Consular officers from the U.S. Embassy in Kyiv will provide services for U.S. citizens in Lviv on October 15, 2025. The services will be available by appointment only between 1:00 pm and 4:00 pm. The specific location of the event will be shared with individuals after their appointments are confirmed.

To schedule an appointment, U.S. citizens must email their full name, passport number, date of birth, and the type of service requested to kyivacs@state.gov or call +38 044 521 5566. The deadline to reserve a spot is Friday, October 10, 2025, at 3 p.m.

The embassy staff stated: "Once your appointment is confirmed, we will send you further instructions, including the location of our services. To avoid delays, please read the instructions carefully."

Available services include U.S. passport applications and document notarizations. Fees for these services range from $50 (2,150 UAH) for notary services per signature to $165 (7,095 UAH) for certain passport applications such as first-time adult passports or replacing lost/stolen passports for adults.

For children under age 16 applying for a passport, both parents and the child must be present at the appointment unless a properly notarized DS-3053 form is provided for a parent who cannot attend.

Payments can be made online through designated pay.gov forms—applicants using Form DS-11 should use www.pay.gov/public/form/start/1274042472/, while those renewing with Form DS-82 should use www.pay.gov/public/form/start/1156527186—and must bring printed payment confirmation to their appointment. Cash payments are also accepted in exact amounts in either U.S. dollars or Ukrainian hryvnia.

Applicants will need to arrange to pick up their new passports at the U.S. Embassy in Kyiv after processing.
