Representatives from the Regional Federal Benefits Unit will visit the Puerto Plata Consular Agency to provide services for U.S. Social Security beneficiaries and individuals with questions about benefits and applications, according to an announcement from the U.S. Embassy in Santo Domingo.

The services will be available at Plaza El Doral, Carretera Luperon KM 3 ½, Puerto Plata, on September 3 from 1:00 p.m. to 5:00 p.m., September 4 from 8:00 a.m. to 12:00 p.m. and again from 1:00 p.m. to 5:00 p.m., and September 5 from 8:00 a.m. to 12:00 p.m.

A limited number of tickets for appointments will be distributed each day on a first-come, first-served basis. Attendees are advised to bring original documents as well as photocopies for their applications.

For those applying for Social Security benefits, required documents include birth certificates, unexpired passports or local IDs, and if applicable, marriage or death certificates for auxiliary or survivor’s benefits.

Applicants seeking a Social Security Number card should bring a copy of their unexpired U.S. passport, original Consular Report of Birth Abroad, original birth certificate, and a completed SS-5FS form.

To change address or banking information related to Social Security payments, an unexpired passport or local ID is necessary.

For Medicare Part B enrollment or cancellation, individuals must complete the CMS-40B form for enrollment or the CMS-1763 form for cancellation.
