Stephanie Syptak-Ramnath Ambassador | U.S. Embassy in Peru

Consular personnel from the U.S. Embassy in Lima are scheduled to visit Cusco's Consular Agency on March 12-13, 2025. The visit aims to offer expanded services, including passport applications (DS-11 only), registering births abroad to U.S. citizen parents, and notarial services.

A limited number of appointments will be available for individuals who submit complete requests via email. Future visits are planned for April and May, with dates to be announced once confirmed.

For passport applications, appointments will be available for adults and minors applying with a DS-11 form. Adults eligible for mail-in renewals must use the mail-in program or arrange other appointments at the Consular Agency. To request an appointment, applicants should send specific documents to CuscoACS@state.gov with "Passport Application CUSCO" and their name as the subject line.

CRBA appointments will document U.S. citizenship for children born abroad to U.S. citizen parents. Applicants need to create a MyTravelGov account, access eCRBA, submit their application online, pay $100 on Pay.gov, and email CuscoACS@state.gov with "eCRBA Appointment Request CUSCO" and the case number in the subject line.

Notarial services can be requested by emailing CuscoACS@state.gov with "Notarial appointment CUSCO" and the applicant's name in the subject line.
